Direct Primary Care Model: Our practice operates on a Direct Primary Care model, which means that patients pay a periodic fee for access to primary care services as listed under the appropriate tabs on the Website.
Services Not Included:
Our services are not a substitute for emergency medical services. In case of a medical emergency, please call 911 or go to the nearest emergency room.
We do not prescribe controlled substances.

Telemedicine Defined: Telemedicine involves the use of electronic communications to enable the Physician to provide certain medical services to patients remotely. Our telemedicine services comply with all applicable Oregon laws and regulations, including the Oregon Telemedicine Act (ORS 743A.058).
Telemedicine Platforms: We use a HIPAA-compliant telemedicine platform, to provide secure video consultations. This platform employs end-to-end encryption and other security measures to protect your privacy.
Limitations: You understand that telemedicine has limitations compared to in-person visits, including:
- The inability to perform physical examinations or certain diagnostic tests
- Potential for technical difficulties that may disrupt or delay the consultation
- Possible need for in-person follow-up visits if the telemedicine consultation is insufficient
Technical Issues: You acknowledge that technical difficulties may occur and interrupt service. If a telemedicine session is interrupted and cannot be resumed, you should call our office to schedule an in-person appointment if necessary.
Privacy Risks: While we use secure platforms for telemedicine services, you acknowledge that electronic communication comes with inherent privacy risks. We will take all reasonable precautions to protect your information, but we cannot guarantee the security of data transmitted over the internet.
MEDICAL RECORDS
Record Retention: We maintain medical records in accordance with Oregon law and regulations, which generally require retention for at least 7 years from the last date of service.
Access to Records: You have the right to access your medical records. To request access, please contact our office in writing. We may charge a reasonable fee for copying and mailing records.
Release of Records: We will only release your medical records with your written authorization or as required by law.
INTELLECTUAL PROPERTY
Ownership: The Website and its entire contents, features, and functionality are owned by the Physician and are protected by United States and international copyright, trademark, patent, trade secret, and other intellectual property or proprietary rights laws.

DISPUTE RESOLUTION AND GOVERNING LAW
Governing Law: These Terms and any dispute arising out of or related to them shall be governed by and construed in accordance with the laws of the State of Oregon, without giving effect to any choice or conflict of law provision or rule.
Dispute Resolution: Any legal suit, action, or proceeding arising out of, or related to, these Terms or the Website shall be instituted exclusively in the federal courts of the United States or the courts of the State of Oregon, in each case located in the City of Portland.
